Nail Surgery

An ingrown toenail occurs when the edge or corner of the nail grows downward into the soft tissue of the surrounding nail fold rather than over it. As the nail continues to grow, it penetrates the skin, causing pain, swelling, redness, and, in many cases, bacterial infection. Some individuals are anatomically predisposed to ingrown toenails due to the natural curvature of their nail plate, whilst others develop the condition as a result of incorrect cutting technique, tight or narrow footwear, trauma, or hyperhidrosis softening the nail fold.

The big toe is most commonly affected, though other toes can be involved. Left unmanaged, the condition tends to worsen progressively rather than resolve on its own.

Nail surgery at West Bridgford Podiatry is a minor surgical procedure performed entirely under a local anaesthetic, meaning you will remain comfortable and awake throughout. The procedure follows these key stages:

  • A local anaesthetic is injected at the base of the affected toe to fully numb the area, though some pressure sensation is normal.
  • Once the toe is fully numb, the offending section of nail (or the complete nail, where required) is carefully removed.
  • A chemical agent, phenol, is applied to the exposed nail bed and nail matrix to permanently destroy the nail-producing cells in the treated area, preventing regrowth.
  • A sterile dressing is applied and detailed aftercare instructions are provided before you leave the clinic.

We provide the following surgical procedures, selected based on individual clinical assessment:

  • Partial Nail Avulsion (PNA) with Phenolisation Matrisectomy: One or both sides of the toenail are removed, and phenol is applied to the matrix to prevent that section of nail from regrowing. The central nail plate remains intact.

  • Total Nail Avulsion (TNA) with Phenolisation Matrisectomy: The complete toenail is removed, typically used for severe or extensively affected nails, or where a severe fungal nail infection is beyond conservative treatment.
Before Your Procedure

Planning your nail surgery appointment is important. Please read the following carefully:

  • Arrange transport home. You must not drive yourself after surgery, as your car insurance will be invalid following local anaesthetic administration to the foot. Please arrange to be collected approximately one hour after your appointment, or ask us to arrange a taxi.
  • Wear or bring open-toed sandals. The post-operative dressing will be bulky and will not fit inside regular footwear.
  • If your work involves prolonged standing, we recommend booking at least one week of relative rest following the procedure.
  • Inform the clinic in advance if you are planning travel or a holiday within the expected healing period.
After Your Procedure
  • Rest as much as possible and keep the affected foot elevated for the first two to three days.
  • Avoid sports and strenuous physical activity for up to three weeks post-surgery.
  • A follow-up redressing appointment is included after your surgery, at which your podiatrist will assess healing progress and provide stage-specific aftercare advice.
  • Full recovery for a partial nail avulsion is typically six to eight weeks. A total nail avulsion may take up to twelve weeks for complete healing.

Nail surgery at West Bridgford Podiatry is conducted in accordance with NICE clinical guidelines, HCPC standards of practice, and the guidance published by the Royal College of Podiatry. Phenolisation matrisectomy is the most widely used and evidenced technique for the surgical management of ingrown toenails, achieving a permanent outcome in approximately 99% of cases. Recurrence rates are lower than those associated with surgical excision techniques alone.

Nail surgery is appropriate for patients who:

  • Have an ingrown toenail that has not responded to conservative management
  • Experience recurrent ingrown toenails affecting the same toe
  • Have developed infection or hypergranulation tissue at the nail fold
  • Have a severe fungal nail infection that is beyond the reach of topical treatment
  • Wish to avoid repeated clinical visits for conservative management of a structural nail problem

For patients with early-stage ingrown toenails, conservative nail edge removal may be attempted first as part of a Routine Foot Treatment appointment. Your clinician will advise the most appropriate course of action at your initial consultation.

Frequently Asked Questions

Will nail surgery be painful?

The procedure itself is not painful, as it is performed under a local anaesthetic. You may feel some pressure and slight discomfort during the surgery. Some soreness in the days following is normal and is typically well managed with over-the-counter pain relief such as paracetamol.

Phenolisation matrisectomy achieves a permanent outcome in approximately 99% of cases. A small number of patients experience regrowth of a narrow nail spicule, which can be retreated if necessary.

This is possible in some circumstances but requires careful consideration of your recovery needs and practical arrangements for getting home. Please discuss this with Hassan at your initial consultation.

Patients in desk-based roles often return to work within two to three days. If your role involves prolonged standing or walking, we advise a minimum of one week of relative rest before returning.

Following partial nail avulsion with phenolisation, the remaining portion of the nail grows normally and can look cosmetically acceptable over time. The treated edge will not regrow. Following total nail avulsion with phenolisation, the nail does not regrow from the treated matrix.

Phenol is a caustic chemical agent used in podiatric nail surgery to chemically ablate the nail matrix. When applied carefully and precisely by an experienced podiatrist, it is a safe and highly effective method of preventing nail regrowth in the treated area.
